Family Care Coordinator (FCC)
About the Role:
The Family Care Coordinator (FCC) leads the High Fidelity Wraparound (HFWA) process, serving as the family’s main point of contact. The FCC facilitates Child and Family Team meetings, develops a family-driven plan of care, and ensures collaboration between families, natural supports, and service providers.
Key Responsibilities:
Facilitate team meetings centered on family voice and choice
Develop individualized, strengths-based plans of care
Coordinate communication across all team members and providers
Connect families to resources and supports
Ensure accountability and follow-through on team action steps
Ideal Candidate:
Strong organizational and facilitation skills
Experience working with youth and families
Commitment to strength-based, family-driven care
Youth Support Partner (YSP)
About the Role:
The Youth Support Partner (YSP) is a mentor with lived experience in youth-serving systems who supports young people in finding their voice, setting goals, and building a positive future. YSPs ensure youth have a strong role in shaping their Wraparound plan.
Key Responsibilities:
Provide peer mentoring and support to youth
Encourage youth voice, choice, and self-advocacy
Help youth identify strengths and set achievable goals
Offer encouragement, guidance, and hope through shared experience
Ideal Candidate:
Lived experience as a youth in systems of care
Ability to relate to and support young people
Belief in youth leadership and empowerment

Family Support Partner (FSP)
About the Role:
The Family Support Partner (FSP) provides peer support to parents and caregivers based on their own lived experience. The FSP empowers families by offering hope, guidance, and advocacy while walking alongside them through the Wraparound process.
Key Responsibilities:
Provide peer-to-peer support for caregivers
Share lived experience to build trust and offer encouragement
Help families navigate systems and services
Support parents/caregivers in self-advocacy and goal setting
Ideal Candidate:
Lived experience parenting a child with complex needs
Strong listening and relationship-building skills
Passion for empowering families and breaking barriers
Minimum requirements all roles:
· First Aid/CPR certification (this can be completed online)
· Proof of General Auto Liability Insurance
· Valid ID/Driver’s license
· Additional requirements
· Obtain your National Provider Identifier if you do not already have one.
· Complete a current state and national background screen as defined in Wyoming Medicaid Rules, Chapter 47, Section 17 (use the search window to locate “Wyoming Medicaid Rules’).
Education and/or Experience Requirements:
Family Care Coordinator (21 years old or older):
§ Bachelor’s Degree in a human services (or related) field, OR
§ Two years of work/personal experience in providing direct services, OR
§ Linking of services for youth experiencing serious emotional disturbance.
Family Support Partner (21 years old or older):
§ High school diploma or GED equivalent AND two years’ work, OR
§ Personal experience with children with serious emotional behavioral challenges.
Youth Support Partner (ages 18 to 26):
§ High school diploma or GED equivalent with behavioral health needs, OR
§ Experience overcoming various systems and obstacles related to mental and behavioral health challenges.
o Respite (21 years old or older):
§ Two years of work, OR
§ Personal experience with children.
